Got a call from my GP's office yesterday. She wants me to cut carbs and fat. My nephrologist has me on a low protein diet and my weight doctor wants me to increase my calories by double. Since there are only 3 macros to choose from (fat, protein and carbs), I'm confused about how to cut all three macros and to simultaneously increase my caloric intake. Oh well, I'm travelling until the end of July, so I'll try to work it out before I get home. I hope y'all have a wonderful weekend.

Sounds like one of them needs to refer you to a dietician to help come up with a proper plan that you understand! Good luck.

Personally I would follow the Nephrologist's advice, since the GP probably referred you to that doctor. A specialist has specifically trained for your condition. But perhaps you could share your labs & medical records with all three doctors so that one doesn't overlook a condition and tell you to do something harmful.

Write a letter to all three and have them duke it out. (Sorry, not in the mood today for conflicting medical advice. Anticipating some soon myself.) Meanwhile do what you can to eat a healthy diet (good fats—not French fries, fruits & veggies for fiber etc.)

My doctor is always on my case she doesn't want me to gain we wait but she doesn't want me to go below 140 don't understand so good luck with your search

I'm with JustBananas. If you have a kidney condition and are seeing a nephrologist, I would think that would be the most critical issue. Did your GP say why they wanted you to cut carbs and fat?

Ask to see a dietician. They can help you plan meals to meet those goals. Ask about how many grams of carbs protein and fat they consider as low.

I'm thinking maybe the GP wants you to cut out less nutritious carbs and "bad" fats, and nephrologist wants you to avoid proteins that are difficult to metabolize. so in theory you could jack up calories by eating like the meditearanean (sp?) diet - it'd just be a lot of food, but it'd all be super nutritious food with the carbs being complex carbs and the fats being good fats like olives and avocados. Does that sound possible?

Awesome. Doctors barking out generalized, confusing, contradicting diet advice. Ask for a referral to an RD, or bite the bullet and book an appointment and just suck it up and pay for it. You need an advocate. What's your current calorie/macro break-down, and what's your cap on protein?

Snowman, with your Stage 4 Chronic Kidney Disease, the Nephrologist's diet of low protein should win the diet dilemma. Does that doctor have any concerns about your carb and fat intake, and how your kidneys are filtering everything? Is there also insulin resistance or high cholesterol to contend with? Maybe ask what you CAN eat and get a list of the off-limits foods. BTW - I hope recovery from your recent accident is going smoothly!

Hi, obviously not sure what’s going on with your health, but often functional medical practitioners are great at getting to the root cause. https://psrmed.com/ It’s virtual. I think they do a hair, blood analysis to find out what’s goin on in your body.

It’s easy enough to google “CDK stage 4 diet recs” to get lists of foods you can and can’t eat. It’s doubling daily calories, while cutting fat and carbs but keeping protein low (Snowman, are you roughly 0.6 g/kg/day or higher?) that’s literally bonkers. The only thing I can figure is that you are so active, your GP may be worried about muscle wasting - which is also dangerous in advanced kidney disease. Again, an RD, especially one who specializes in renal nutrition, will be able to look at your data and calculate exact macros that satisfy your needs.

I also vote for exploring functional medicine AND looking for a functional/holistic RD if it is within your means. Safe travels👍.

Def voting for the RD and coordinating with all three docs. Ignore the FS guidance for this type of stuff. Too risky.
